New Amsterdam begins tonight with Dr. Max Goodwin (Ryan Eggold) laying down, as Dr. Helen Sharpe (Freema Agyeman) gives him the 3-dot tattoo which will guide the external radiation beam. He jokes about wanting a badass tattoo instead. He doesn’t want the staff to know about his treatment, but she reminds him they will notice. EMTs arrive with Dr. Lauren Bloom (Janet Montgomery), who thanks them for the ride from the Bronx; Casey (Alejandro Hernandez) is surprised she is pulling a double again but doesn’t say anything.

Evie (Margot Bingham) catches Lauren in the hallway, asking if she knows what kind of scotch Dr. Floyd Reynolds (Jocko Sims) drinks, revealing it is his birthday. Lauren wishes her luck, saying she will need more than scotch to please him.

Max finds Floyd bringing a machine to his OR, saying “finders keepers” but is unable to move it; Max walks away. Dr. Vijay Kapoor (Anupam Kher) talks to Rohan (Vandit Bhatt) about the game, but Rohan turns him down about coming to the game. He tells his dad that he is getting his 1-year chip for being clean and it’s a big deal. Kapoor feels being clean should be the reward and offers for them to go to the game next week instead; its clear Rohan is disappointed as Kapoor is called off to work.

Helen talks to Max about a patient who has pancreatic cancer but cannot suggest the surgery because of a heart condition they found. Max says they need to give her patient both options. Dr. Sharpe informs him his heart isn’t pumping blood very well and 90% chance of him dying in surgery or he can have no surgery and go home, spend a good year of his life with the people he loves. He asks for some time to think about this; Max asks him to really consider both options.

Dr. Kapoor meets with Amy, saying they have exhausted all traditional treatments; her mother wants them to find something out. He suggests it could be stress and counseling as an alternative treatment. He gives them Dr. Iggy Frome’s (Tyler Labine) card, but her mother takes it away, saying it is nonsense as she has headaches, but Amy feels one appointment cannot hurt and maybe Dr. Frome could help. Her mother doesn’t want her to talk to strangers about their problems and Amy walks right in front of an oncoming train as her mother screams.

Amy is brought into the hospital, where Dr. Bloom learns from Casey she was there that morning to see Dr. Kapoor. Iggy explains to Max what is going on with Amy’s case thus far; Max says in some Asian communities depression is hard to accept. Iggy fears if they don’t help her, she will try to kill herself again.

Evie calls Floyd a “Birthday Dork” when he opens the machine, he realizes it is just a massive waste of money. Max visits Helen, who is looking at potential sperm donors; she says she is giving her patient time to think about his decision. He doesn’t want to wait, saying a year with cancer is not something people would want. Dora (Zabryna Guevara) gets him to sign some papers, asking her to settle the debate; she hears both sides and can’t choose. Max is shocked when he finds out that the Rabbi is definitely not doing the surgery, feeling this is the path God has chosen for him and he wants the year. Max struggles with the decision.

Iggy and Kapoor sit with Amy’s mother, very sorry for what happened to Amy. They want to discuss inpatient psychiatric care, but she insists Amy isn’t crazy. Iggy says she only needs help and they want the best for her. Amy is the patient and jumped in front of a subway train; her mother says she didn’t jump, she slipped. In the ED, Casey tells Lauren that he knows doctors who work more than 30 hours or more make 36% more mistakes. She says she is Thomas Edison and thrives off short naps. He finds a typo in her report, so she takes a pill and shakes her head, continuing back to work.

Max talks to the Rabbi about his decision, not as a doctor, wondering how he is at such peace knowing he is going to die. The Rabbi explains the story of Abraham and Isaac, trusting in his faith willing to kill his own son to have many more; what if Abraham said no as he’d rather have only Isaac and the rest be damned. He simply chose the one that makes him happier. Max reveals he has cancer and chemo starts tomorrow. He is scared, frustrated and learning he isn’t a good patient as he is also angry, not sure what to do. The Rabbi tells him cancer changes everything and nothing at all.

Lauren can’t deal with all the sounds of the ED, a nurse gives her a tablet where she is told her patient, Roger’s asthma is better. Lauren says since he has hypertension they are going to have to watch him overnight. She reassures the wife he is going to be okay. Lauren hands Casey the tablet, saying she is going to hit the on-call room for a nap.

Evie tells Lauren she has decided to throw Floyd a surprise party after work; she continues to ask questions but just as Lauren lays down she is called back for a consult on another patient. Iggy is able to see Amy after surgery, saying she is very lucky to be alive. Iggy asks her routine questions and she admits she has stress and she can’t have depression as she has many good things in her life. He tells her that depression is highly treatable, but the first step is to ask for help. Amy worries that if her family finds out, they would believe she is weak. She refuses to talk since she is under their insurance and her parents would think she is revealing family secrets and would disown her.

Iggy, Kapoor, and Max discuss Amy’s treatment and the stigma and shame. Max tells Iggy to defraud the hospital to help Amy. Helen rushes after Max, wanting to know what he said to the Rabbi because he has changed his mind and now wants the surgery. He tells Max it is tough luck that he tipped the scales. He could be home with his family, but he’d still be a patient. He is going to do this, either he is going to survive or he will die and no longer be a patient. Before the Rabbi goes into surgery, he tells Max that Abraham didn’t have to sacrifice Isaac as it was a test. God rewarded him with many, many children and some things are worth the risk.

Max also runs into the cell sorter, complaining it is in the hallway; Floyd explains it won’t hook up to any other machines but no one will take it back. Sandra from billing stops Max, asking if he is aware that two of his doctors are downloading; he reveals that he told them to do it as they are doing it to help a patient. She says her job is to make sure the hospital has enough money to help all their patients and wants to know how he plans to make up for the loss of revenue.

Amy learns from Iggy how they are going to help her without her family ever finding out. Iggy says it is a lie but on paper, as they put it down as brain stimulation. Amy feels her mother and father will judge her and not love her the same and doesn’t think Iggy can teach her a coping mechanism for that. Iggy tells Kapoor the only way this is going to work is to get her mother on board, or they will lose Amy.

Casey wakes up Lauren, saying Roger went into afib. She says she is fine, but when they ask her what to do, she freezes. She finally tells them which drug, insisting the med will work. Casey and another doctor prepare to shock him but she orders them to stop and give the medication a chance to work, which it does. Roger wakes up as Lauren tells him he is okay. Lauren walks back to the on-call room when Evie asks her more questions about Floyd, she finally says she doesn’t know how old he is going to be; they only slept together a few times.

Kapoor talks to Amy’s mother, trying to get her to admit Amy didn’t slip and it’s not that she is a bad parent. He says their children are not perfect and why should they be since they are parents aren’t either. Max stands with the Rabbi’s wife, who believes he was always going to do the surgery, but needed to explore the alternatives; and she loves him for it.

Iggy returns to Amy’s room. She says she feels like she has been hit by another train; which he says is a good thing as humor is great. He talks about being from a good family and tight community. She says the hardest thing about every day is just getting out of bed, thinking about all the things she needs to get done that day and picturing herself failing at every single one; ultimately feeling like she shouldn’t try. She just wants to tell her mom how much pressure she feels trying to make her proud. Her mother hears it all and Kapoor says that her mother knows how they can bill the insurance. Her mom tells her she wants her to get better as Amy says she needs help. Her mother says they all need help.

Floyd and Evie walk into his surprise party, where he thanks her for being so amazing. Iggy pages him over to the karaoke machine; he wants to know how she knew and she reveals Bloom told her. Lauren lays in the on-call room, exhausted. Kapoor arrives to see his son, Rohan receive his chip, very proud. Amy enjoys it as her mother brushes her hair.

Max watches as they perform surgery on the Rabbi, seeing himself in the reflection. He returns to Helen’s office to tell her the surgery is going well. Helen reveals she still misses Mohammed, one of the good ones; they were over their minds for each other. They were going to get married and have kids, but he had a brain aneurysm and all their plans gone and now she is here freezing eggs, asking strangers for help. She feels like she is betraying him and doesn’t know if it is worth it but Max tells her it is. They hear everyone singing at Reynolds’ birthday and feel they should stop him before he hits the punk rock.

Max tells her to cancel chemo as he wants to try precision target therapy because chemo is going to take the life out of him and he wants to continue running the hospital as well as being there for Georgia and the baby. He doesn’t want to be a patient, he wants to be himself. She worries it is too big a risk but it’s one he is willing to take.

THE END
