A bus has gone through a guard rail. Mag and Bashir head to the scene. They arrive to find chaos. They begin to triage their patients while Bashir helps a kid named Lucas who has possible fractured vertebrae. He then finds someone who cannot speak. Her name is Emily. She has a piece of glass next to her jugular. She is hemorrhaging. Mag comes to help. He takes the shard out and stops the bleeding with saline and a catheter.
On the way to the hospital, Emily’s friend rides in the ambulance. She may have a concussion. Bashir gets them into the hospital, giving a report. In one of the bays in the ED, Bashir meets the acting chief who isn’t happy he used a balloon to stop the bleeding next to Emily’s jugular. It was protocol and it was ballsy.
Meanwhile, June places a chest tube in a young man named Lucas who has a pneumothorax. He tells her his feet are tingling. They notice a positive Babinski sign. They run a CT and see he has internal decapitation.
Bashir is told the driver Ally may have been drinking. He talks with her. She denies it and says she is allergic. She gets very angry and defensive. Mag looks over a girl named Lizzie. She calls Dr. Hunter in. The young lady is a hemophiliac. The acting chief gets involved. Mag has been one for two days straight. She needs to go. Bashir runs into the chief again. He somewhat apologizes for his shortness. The chief doesn’t seem to care.
June helps Lucas stay calm as he cannot move while Bashir tends to Ally who suddenly feels like her throat is full. Emily’s mom is there. She learns Ally has been drinking. Ally swears she was not drinking. The tests are wrong.
June is asked to come to the OR with Lucas. Bashir talks with his family while on break. He wants his daughter to get rest for school tomorrow. He promisees to see them in the morning. Emily’s mom is told that her airway is damaged and the next few hours are critical.
Mag brings Lizzie her belongings. Lizzie starts to be short of breath. They see she has a clot. She needs a clot buster. They give her one. Mag waits with her as she is anxious. Bashir learns that Ally has a GI condition after taking a long course of antibiotics. She wasn’t drinking. He heads to tell Emily’s mom who isn’t very forgiving. Bashir tries to help her understand.
Mag gets in trouble with Theo for giving the clot-buster since it wasn’t her patient. The chief comments on Bashir’s skills from overseas. Mag heads to the new chief who tells her it was fine she gave the clot-buster. If she wants to stay late with no sleep that’s her problem.
Bashir is in the room helping when Emily takes some breaths on her own. Ally watches from the outside as Emily’s mom kisses her. Bashir runs after Ally to cheer her up. Later, Jed comes to. Bashir is happy to hear his voice. He catches up with Mag after. She asks him to go for a drink but he cannot. Bashir heads to the motel to see his family. His ex tells him what really happened. She stayed away so she wouldn’t ruin his new life. He hugs her.
