Noah begins to doubt his abilities as a doctor. Dr. Marcel and Dr. Choi are faced with a tough ethical decision. Dr. Halstead puts his faith in an addict.

Chicago Med begins tonight with Dr. Will Halstead (Nick Gehlfuss) waking up and kissing Dr. Hannah Asher (Jessy Schram). He has to leave because his shift starts soon but wants to make sure she is still going along with the plan to report herself; she asks him if he wonders that she slept with him just to get out of it. She says she will round her patients and hand them off; then she will go see Sharon Goodwin (S. Epatha Merkerson), wanting Will to go with her.

Dr. Natalie Manning (Torrey DeVitto) comes to see her first patient and his daughter, Harper Beck (Chandler Head) explains that her father is experiencing belly pain. Natalie asks for various tests, promising to Harper that she will do everything they can. His ex-wife tells Manning that everything Lewis said is not true. His real name is Derek Jordan, an actor from New York. She hired him to play the role of her daughter’s father.

Manning returns to speak to Derek, asking Harper to go outside for her to gather some real information. His eyes are awfully yellow so Natalie reminds him that she needs his medical record and not the characters. He insists she is mistaken as his name is Lewis Sharron and he has no idea what she is talking about. Outside the room, Natalie asks Doris to run a bunch of liver tests and to page Dr. Daniel Charles (Oliver Platt) as she is sure this guy is nuts.

Will hands a file to Maggie Lockwood (Marlyne Barrett), as she reveals her hormone therapy is making her feel like her body is crawling with ants. He encourages her to hang in there and wait for her levels to even out. They are interrupted when a very pregnant patient, Sienna Fredericks who is having a pain crisis; her OB is Dr. Hannah Asher. She demands no morphine, explaining that she got pregnant with the third round of IVF and this baby doesn’t have the disease. She wants them to call down Asher as she will know what to do. She doesn’t answer her phone so Will goes up to find her.

Natalie speaks to Charles, wanting to know what they should do. Just then Dr. Ethan Choi (Brian Tee) and nurse April Sexton (Yaya DaCosta) walk into the ED, quite lovey-dovey with each other. Maggie quickly asks Dr. Noah Sexton (Roland Buck III) and April can handle a patient who is refusing to go in the ambo.

Natalie and Dr. Charles explain to Elaine Beck (Stephanie Lemelin) that Derek is very sick, but are unable to make an accurate diagnosis. She wishes she could help but this is how he is. Natalie threatens to call CPS but Elaine said she did this to Harper because she herself didn’t grow up with a father and Harper needed it. She explains how she keeps up the rouse, but did tell Derek this is the last time as he will return to Geneva as Lewis and stay extremely busy, losing tough with Harper; eventually, she will tell Harper that her father died. She doesn’t feel this is traumatic for Harper and wants them to figure out what is going on with Derek. Charles wonders if Derek knows his character is going to die, is this a coincidence?

Will barges into Hannah’s office, learning they canceled her first appointment. Meanwhile, April and Noah run with their patient who was told to keep his heart rate up and call 911. Noah and April decide to give him an EpiPen; but as soon as they give it to him, he collapses making his heart drop. He has to keep running as April thinks of a new idea.

At Chicago Med, Ethan and Dr. Crockett Marcel (Dominic Rains) met their patient Walter who is a prisoner in custody. They find out he was set for execution and did receive the lethal injection but it didn’t kill him.

Sharon feels the guards used something without the pharmaceutical authorization as they don’t allow their drugs to kill people anymore. They put the needle into his muscles, missing his veins completely; both Ethan and Crockett agree to treat the guy and get him the hell out of there.

Will returns to see Sienna, informing her that Dr. Asher is unavailable and they have another OB. She gets frustrated knowing the doctors are only going to assume she wants the drugs. Maggie says she needs the transfusion if she wants the baby to survive; which she agrees to. Will places a call to Hannah, saying she has a patient who needs her and for her to call him and let him know she is alright.

Larry is on treadmill and April tells him he has no choice but to keep running as his heart rate is extremely low. Noah says he needs to call MED as they need a real resident there. Daniel reads a story to Derek about who Lewis is supposed to be. He feels it is wrong the way Elaine wants him to die and Lewis would never do that. Derek plans on dying in front of Harper, so it will give her the closure she deserves. Natalie is astounded and wants to tell the young girl unless he explains what poison he took and he refuses.

Ethan and Marcel’s patient wants to die. He deserves to die but not like that as he felt everything go on fire through his whole body. Sharon learns that the patient is stable and now he needs to return to the prison as they have already rescheduled his death sentence. Marcel and Ethan seem to disagree with the situation.

Maggie wants Noah to push a wire into Larry’s heart as he doesn’t have much more time to run. Shew agrees to help him.

At the ED, alarms sound and Maggie rushes over to see Sienna is having complications and the baby isn’t getting enough oxygen, She is 7 months pregnant and orders no one to lay another finger on her. Will instructs Maggie on what to do as he believes he knows where he can find Dr. Asher.

Larry continues to job on the treadmill as April explains what they are doing to him. It seems Noah overshot the artery and needs to start over again with the wire. April gives him chest compressions while Noah begins with the wire again. Elaine is shocked that Derek aka Lewis would poison himself because he needs to die in front of Harper in order for her to never wonder. Elaine is more concerned about Harper finding out the lies she did. She agrees to say as long as Derek agrees because Harper won’t believe them otherwise.

Will races to the safe injection site, saying he is looking for one of their regulars. Will is unable to tell them anything. Noah can’t advance the wires to his heart as April gets another one set of hands to do the chest compressions. Together April and Noah get it in, but there is still nothing; suddenly Larry begins to cough and they inform him that he will be transported to Med.

The prison inmate had so much blood loss in his one arm that Choi feels they need to amputate but Crockett says there is no point in amputating a man’s arm who is about to be executed. They stand toe to toe opposed to this as Choi reminds Crockett that they are doctors and not executioners. Will informs Sienna that she and her baby are not getting enough oxygen and if she doesn’t have the c-section soon, both of them are going to die. She doesn’t need to trust him but she will soon lose consciousness and the hospital will do a c-section anyways. Maggie talks sternly to her, pleading for her to let them help her. She nods and Will calls for an OR.

In the OR, Ethan and Marcel amputate the prisoner patient but Marcel notices more of an issue and they have to advance into the shoulders and neck. Meanwhile back in the ER, Elaine rushes to Derek’s side, who is now extremely sick and also won’t change the story. Natalie reminds him his kidneys are starting to shut down and wants to know what poison he took. Dr. Charles suggests staging his dead, which gives Harper a final farewell and he will just fall asleep. He promises to tell them what he took after they stage his death. Natalie says Charles cannot do this but he reminds her that one way or another Harper is going to face her father’s death and this way will be less traumatic to her and they can then save Derek’s life.

Ethan and Crockett continue to struggle through surgery as Ethan stands back watching Crockett doing the best he can. They lost his pulse and as the nurse says they need to start compressions, Ethan says all his blood is on the table; Crockett announces the time of death.

Harper visits with her daddy, who explains he doesn’t think the hospital can fix him. He tells her that he is so proud of her and he loves her more than anything. They two exchanged hugs as Daniel suggests they all give her daddy some rest. She is about to run out the door but runs back in, shouting that he is going to get better as they are going to do everything they said they would do. Derek opens his eyes and tells her that he is okay. He informs the hospital staff that he swallowed cyanide and he will be okay. Natalie nods to Daniel as their version of the plan worked.

Sienna’s c-section is happening, as Will just observes. Premature baby girl is born but she is healthy. Will takes a sigh of relief when he learns mom is fine too. He is walking down the hall when Asher arrives demanding to know how her patient is. Will confronts her, feeling she is lying about what happened to her that day. She brushes past him to see Sienna; Will is very stern.

The prison guards leave, saying the prisoner’s death is on their shoulders. Sharon wants to know if there is anything she needs to know about the surgery as Ethan assures her they did everything they could in there.

The Patient, Larry had a pacemaker embedded into his chest in time and is expected to make a full recovery. They were told that given the circumstances it was an impressive save. Daniel, Natalie, and Elaine sit with young Harper and explain the truth about her daddy. She goes to see him with her mom. Ethan and April had plans that night but he had hoped they could grab a drink with Crockett first, as they both had a rough day on the case today. She suggests Ethan can go ahead, but he changes his mind as they have a date tonight and they leave together.

Will finds Earl, the security guard and needs a favor. They take the drug-sniffing canine through the parking lot and it shows that Will was right. Just at that moment, Hannah Asher walks bout of the hospital.
